We are hiring a part time position for Physical Therapist to join established and rapidly growing Home health Care agency. Excellent pay, set your own schedule and mileage reimbursement. This Physical Therapist is to perform admissions and visits for seniors in Home Care setting. The ideal candidate is a skilled professional that is passionate about delivering quality care and making a real difference.

Responsibilities:

  • ​​​​​​Assess each patient and develop an appropriate treatment plan with measurable goals and time frames.
  • Prepare and submit clinical and progress summaries based on identified and attained goals.
  • Ensure that other treating medical professionals have the necessary information to appropriately evaluate the patient.

Qualifications:

  • Must be license in the state of Michigan
  • Previous experience in physical therapy, rehabilitation, or other related field
  • Good assessment skills, ability to work independently and well with interdisciplinary team required.
  • Must be willing to travel within Berrien county.
  • Ability to handle physical workload
  • Ability to build rapport with patients
  • Excellent written and verbal communication skills
  • Strong leadership qualities
