OB DESCRIPTION OF LICENSED VOCATIONAL NURSE
Ontario Holt Dialysis Center is seeking an LVN to join our team.
Monday, Wednesday, Friday
Time: 6:00 am - 7:00 pm (closer)
Full time * Benefits after 90-day probationary period.
I. Function:
. The Licensed Vocational Nurse performs patient care and related duties as assigned by the Charge Nurse. Must have Hemodialysis Technician License.
II. Qualifications:
a. Graduation from an accredited school of practical nursing.
b. Current licensure as an L.V.N. in the state of CA with I.V certified.
c. Satisfactory professional references.
d. Has completed the dialysis training program/CHT License required.
e. Maintains CPR certification.
III. Duties and Responsibilities:
a. Assign staff to patient care and other duties with in the scope of their training and capabilities.
b. Practice at all times under the supervision of an R.N. or M.D. and within limitations established by law, regulation, or usual practice within the state.
c. Supervise and prepare the dialyzer and delivery system for use as prescribed; monitor all equipment in use for correct functioning; and perform appropriate cleaning following each treatment.
d. Supervise and assess patients before and after each treatment. Initiate, monitor, and terminate dialysis treatment. Consult with the R.N. on all aspects of patient care and execute the plan of nursing care for each patient as directed. Notify the R. N. regarding changes in the patient's condition.
e. Conform to the established policies and procedures of the facility.
f. Perform patient care or other duties as assigned by the Charge Nurse or Head Nurse.
g. Observe and receive reports from staff members on the physical and emotional status of each patient.
h. Make rounds with the physician and ensure that physician’s orders are executed as written.
i. Recognize and is able to respond to any emergency situation, knowledgeable in cleaning and sterilization.
j. Assist in the documentation in the medical record at least monthly progress notes, which reflect all, nursing care provided. These shall include identification of current problems, goals, and actions planned to achieve goals, and an evaluation of patient progress.
k. Safely administers prescribed medications and treatments and takes appropriate action as based on patient’s response.
l. Review monthly home medication.
m. Assist with the instruction and orientation of personnel.
n. Demonstrate good communication skills.
o. Assist in admission and orientation of new patients, facilities transfer, and appointments.
Job Type: Full-time
Pay: $26.00 - $28.00 per hour
