Job Description:

California Nurses Registry is seeking a skilled and compassionate Registered Nurse (RN) to join our team. The RN will perform follow-up visits and admission visits, ensuring high-quality care and support for our patients.

Key Responsibilities:

  • Conduct follow-up visits to monitor patient progress and adjust care plans as needed.
  • Perform admission visits, assessing patient needs and establishing individualized care plans.
  • Collaborate with healthcare team members to provide comprehensive patient care.
  • Educate patients and families on health management and care procedures.
  • Maintain accurate and timely documentation of patient interactions and care plans.

Qualifications:

  • Current and valid RN license in the state of California.
  • Strong clinical assessment and communication skills.
  • Experience in home health or a similar setting preferred.
  • Ability to work independently and manage multiple tasks.
  • Compassionate and patient-focused approach.
