Care Coordinator Job Description.
A Care Coordinator’s job is to supervise the patient’s chronic conditions on a monthly basis in accordance with the provider’s plan of care. This individual is responsible for monitoring and managing the care provided and delivered by the different programs of the company.
Duties & Responsibilities
As a Care Coordinator, you will assist the physician and their team/office staff with any patient needs or updates that associate with their health or healthcare information.
Specific duties:
- Assess and screen patients in accordance with their providers care plan on a monthly basis.
- Perform an intake/update assessment which includes, but not limited to:
- Obtaining and reviewing prior medical records, including specialist
- Medical or program-related financial/insurance data
- Update insurance, verify insurance, and take payment information
- Documenting a complete medical history based on that month’s conversation
- Identify patient symptoms, signs, and disabilities
- Monitor for changes in health status after initiation of a new medication, a hospitalization or recent decline in function.
- Assessing cognitive/verbal skills and needs
- Identifying barriers to accessing healthcare
- Assisting with, but not limited to:
- Helping patient understand or provide orders indicated by provider
- Schedule laboratory testing, immunizations, x-ray appointments, etc.
- Creating/providing referrals for preventive health needs indicated by provider.
- Follow-up with patients when barriers to referrals are identified.
- Assist with arranging supportive services as needed
- Educate and monitor patients lifestyle factors affecting health – such as tobacco use, substance abuse, nutrition and physical activity – and assist the patient with goal-setting to achieve behavioral change.
- Assess and screen patient
Facilitate response to urgent telephone calls, requests or visits from assigned patients.

Monitor
At Complete Care Partners, a Care Coordinator will follow along with our physician's patients monthly and review their healthcare plan to them through telehealth services. They will help the patient with any prescription refills needed and/or doctor's, lab/diagnostic procedures, etc. Care Coordinators become the patients "personal healthcare assistant" and allow them to improve the patient's quality of life by taking care of their healthcare needs. Complete Care Partners follows the Chronic Care Management (CCM) services created by Medicare.
Medical Assistant or Medical terminology needed. Bilingual, need to be able to type at a fast pace, be able to communicate the healthcare plan and explain diagnosis to a patient over the phone. Have a general sense of prescription drugs and diagnosis codes. Also, the individual must be able to thrive in a fast-growing environment.
