Welcome to SoCal Golden Care Home Health

We are seeking a compassionate and hardworking LVN' s interested in Shift Care to provide skilled nursing to patients within their home and clinical work. Our ideal candidate must be compliant and punctual with shifts and electronic charting. Must have good communication skills with staff and multidisciplinary team is required. Candidates must have a reliable method of transportation and have open availability.

  • Variety of assignments, procedures, and treatments
  • One to one patient care with a customized Plan of Care for the patient

Responsibilities

  • Help clients take prescribed medication
  • Assist clients with ambulation and mobility around the house or outside
  • Assist clients with personal care and hygiene
  • Plan and prepare meals with assistance from the clients (when they are able)
  • Be a pleasant and supportive companion
  • Report any unusual incidents
  • Act quickly and responsibly in cases of emergency

Skills

  • Proven experience as an LVN.
  • Excellent knowledge of emergency response and first aid (CPR).
  • Knowledge of housekeeping activities and cooking with attention to dietary constraints
  • Willingness to adhere to health and safety standards
  • Respectful and compassionate
  • Good time management skills
  • Outstanding communication and interpersonal skills
  • Strong ethics
  • Physical endurance

QUALIFICATIONS & REQUIREMENTS Successful candidates must meet the following requirements:

  • Pre-hire TB Test
  • Pre-hire Physical Exam
  • Proof of Covis-19 Vaccination
  • Current CPR (Valid) certification
  • Must be a License LVN/LPN
  • Current California Driver’s License
  • Proof of eligibility to work in the US
  • Ability to lift and/or move up to 50 pounds
  • Reliable transportation to and from the worksite
  • Successfully pass annual competency checks
  • Must approve the request for background and criminal record check.
