Job Summary

As an Entry-Level Field Care Coordinator, you will play a key role in building and maintaining relationships with healthcare providers and facilities in our community. You will work closely with our experienced team to learn about hospice care services, develop outreach skills, and help increase patient referrals. This is an excellent opportunity to start a rewarding career in healthcare sales and make a meaningful impact.

Key Responsibilities
  • Assist in Relationship Building: Support the development and maintenance of relationships with AL facilities, group homes, SNFs, and physicians. Learn to effectively communicate our hospice services.
  • Participate in Outreach Activities: Attend networking events, community fairs, and in-services to promote our hospice care. Assist in organizing events and providing information to potential referral sources.
  • Support Referral Management: Help manage referral inquiries and follow up with potential referral sources. Work closely with the clinical team to facilitate smooth patient transitions.
  • Learn and Advocate: Gain knowledge of hospice care and patient needs. Advocate for patients and families by providing accurate and compassionate information about our services.
Qualifications
  • Passion for Healthcare: A genuine desire to help patients and families navigate end-of-life care.
  • Communication Skills: Strong verbal and written communication skills. Comfortable speaking with healthcare professionals and the public.
  • Interpersonal Skills: Friendly, approachable, and able to build rapport with diverse groups of people.
  • Education: Bachelor’s degree in healthcare, marketing, communications, or a related field is preferred but not required. Recent graduates are encouraged to apply.
  • Willingness to Learn: Open to receiving training and guidance from experienced team members.
