- Care Coordination:
- Develop, implement, and regularly review individualized care plans for the TBI patient in collaboration with healthcare professionals, the patient’s family, and the care team.
- Schedule and coordinate care team members, ensuring coverage and continuity of care.
- Serve as the primary point of contact for the patient and their family regarding care-related concerns and updates.
- Supervision and Support:
- Oversee the performance and activities of STNAs and Medical Assistants, providing guidance and support as needed.
- Conduct regular assessments and evaluations of care team performance, providing feedback and training to ensure high-quality care.
- Address any issues or concerns related to the care team’s performance or patient care.
- Medical and Administrative Duties:
- Monitor and document the patient’s progress, including changes in condition, response to treatment, and any new medical needs.
- Ensure that all care documentation is accurate, timely, and compliant with healthcare regulations and agency policies.
- Coordinate with external healthcare providers, including physicians, therapists, and other specialists, to ensure integrated care.
- Quality Assurance:
- Implement and adhere to quality assurance protocols and best practices to ensure the patient’s care is safe and effective.
- Review and update care plans and protocols as necessary to adapt to the patient’s evolving needs.
- Handle any complaints or concerns from the patient, family, or care team promptly and professionally.
- Education and Training:
- Provide ongoing education and training to the care team on relevant topics, including TBI-specific care needs, safety protocols, and any new procedures.
- Keep abreast of developments in TBI care and home care practices to ensure the care team is knowledgeable and up-to-date.
- Emergency Response:
- Develop and review emergency care plans, ensuring all team members are aware of procedures and protocols.
- Respond to emergencies effectively, providing guidance and support to the care team and coordinating with emergency services if needed.
- Education: Bachelor’s degree in Nursing, Social Work, Healthcare Management, or a related field preferred. Relevant certifications or licensure (e.g., RN, LSW) are advantageous.
- Experience: Minimum of 2-3 years of experience in case management or related roles, with a preference for experience in home care or working with patients with traumatic brain injuries.
- Skills:
- Strong organizational and multitasking skills with the ability to manage and prioritize multiple responsibilities.
- Excellent communication and interpersonal skills, with a proven ability to work effectively with patients, families, and care team members.
- Proficient in care coordination, medical documentation, and regulatory compliance.
- Knowledge:
- Familiarity with TBI and its management, including common challenges and care strategies.
- Understanding of home care regulations and standards.
- Background Check: Successful completion of a criminal background check and other required screenings.
- Transportation: Reliable transportation with a valid driver’s license and proof of insurance, if travel is required.
- Competitive hourly rate based on experience and qualifications.
