Overview: YAI’s California Services is is looking to hire an Intake Specialist for the California Region. The Intake Specialist supports YAI California residential services, and other programs as needed, to manage vacancies and streamline point of entry for referrals from the Regional Centers.

Job Description / Roles/ Responsibilities:

· Receive and review referral packets for YAI services (Residential, and others depending on regional needs);

· Use clinical knowledge to provide effective screening and refer people to YAI screening team;

· Work with Regional Centers to request required documents for assessment and enrollment;

· Collaborate with program management throughout screening process and ensure adherence to screening process timeframes;

· Maintain data and documents in shared drives and appropriate database systems;

· Verify each applicant’s eligibility and ensure necessary enrollment codes are present upon admission;

· Work with leadership in each region to help plan for future needs in existing opportunities and new development;

· Establish and maintain linkage with community partners (DDS, Regional Centers, private providers and County Behavioral Health.

· Participate in outreach and community-based meetings to network, present YAI, and receive referrals;

· Acquire knowledge about the DDS and Regional Centers systems, eligibility, referral procedures, benefits and entitlements, compliance, and the vast array of services available to people with I/DD;

· Complete other tasks based on departmental and regional needs and as assigned by supervisor.

Qualifications:

· BA/BS Degree preferred

· A minimum of 2 years of experience working with the I/DD population or other experience in Social Services

· Comfort with conducting screenings in hospitals, nursing homes and other non-YAI settings as needed;

· Experience in Service Coordination/Case management or Intake/Admissions preferred

· Must be able to communicate clearly and effectively, verbally and in writing, in order to best represent YAI.

· Must be able to work autonomously and work well in groups.

· Must be proficient in basic computer skills including Microsoft Office 365 and the applications; word processing and data entry;

· Able to learn and adopt to many systems including Salesforce, Outlook/OneDrive, eVero DA and IATS among others.

· Highly organized, good time management skills and ability to prioritize multiple tasks.

· Local travel required to conduct tours and screenings.
