We are seeking to hire a seasoned and reliable community health worker to perform various functions related to preventative quality measures at a growing family practice medical office in Victorville. If you are dedicated and detail oriented, our local family practice has a place for you to work with a collaborative, patient centered team that has been recognized as “best of the desert” and serving the High Desert since 2005.
Our community health worker will work directly with providers and patients while relating exceptional patient care. This is a great opportunity for someone who cares for people and is attentive to detail.

Job Summary
Under the supervision of the medical provider, the Certified Community Health Worker (CCHW) will play a crucial role in improving the health outcomes of our patient population by providing culturally competent and linguistically appropriate health education, navigation,
support, and advocacy. The CCHW will collaborate with the healthcare team to identify and address patient needs, facilitate access to care, and promote health and wellness within the community. This position adheres to the guidelines of Molina Healthcare of California’s CHW Medi-Cal Benefit.
Essential Duties and Responsibilities
● Conduct health education sessions on various topics such as chronic disease management,
preventive care, nutrition, and wellness.
● Provide individual and group health counseling and support to patients.
● Assist patients in navigating the healthcare system, including scheduling appointments,
completing forms, and accessing community resources.
● Identify and address social determinants of health that impact patient outcomes.
● Conduct health screenings and assessments to identify patient needs and develop care plans.
● Advocate for patients’ rights and needs to ensure access to quality healthcare.
● Build relationships with community partners to enhance service delivery and resource
availability.
● Maintain accurate and up-to-date patient records and documentation.
● Participate in staff meetings, training, and quality improvement initiatives.
Qualifications
● Certified Community Health Worker (CCHW) certification required.
● Bilingual (English/Spanish) preferred.
● Strong interpersonal and communication skills.
● Ability to work independently and as part of a team.
● Knowledge of community resources and social services.
● Experience working with diverse populations.
● Proficiency in electronic health records (EHR) systems.
Benefits
● Hourly rate: $20 per hour
● Hours: Monday-Thursday, 8:00 AM – 5:00 PM; Friday, 7:00 AM – 3:30 PM
