Job Overview:


Wasatch Crest Treatment Services is actively seeking a dedicated Case Management Coordinator to join our team. Case Management works to ensure that each client receives the support and information necessary to build a life in long-term recovery. The Case Manager is responsible for creating and developing discharge and aftercare plans, referent communication, family communication, and transitions through a full continuum of care. It is their responsibility to ensure that clients, referents, and families receive world class hospitality and care.

 

Duties and Responsibilities:

 

Program Development

  • Develop and implement client programing as relate to case management.
  • Support and direct daily functions of case management.
  • Accurately maintain required documentation.
  • Participate in weekly clinical meetings.
  • Review case management assessments and treatment planning.
  • Support team in facilitating internal and external goals.
  • Develop and build meaningful relationships with other treatment programs and professionals.

 

Client Care

  • Meet weekly with each client to develop aftercare plan and educate on the importance of continuing care at Wasatch Crest.
  • Communicates with referents and families in a professional timely manner.
  • Communication is handled with impeccable follow through as requested by referents and families.
  • All clinical communication requests will be communicated and documented.
  • Complete Case Management Assessment and create treatment plan based on findings.
  • Ensure completion of Uinta Application and communicate as needed.
  • Complete Uinta rent agreement and coordinate with team to ensure payment each month.
  • Ensure clients and families understand financial responsibilities. Assist in collection of payments. (Continuing care, cost, resources needed….)
  • Provide information, encourage, and create excitement for all clients to complete the full continuum of care at Wasatch Crest.
  • Coordinate transportation if needed.
  • Monitor clients physical and emotional well-being and report unusual behavior or physical ailments to clinical staff.
  • Collaborate with or assist clinical team to treat, rehabilitate, and return client to the community.
  • Provides high-end hospitality with professional, attentive, and courteous service that ensures the client welfare and comfort.
  • Responds to client complaints immediately and reports relevant concerns to supervisor.
  • Meet or exceed primary goals as listed below.

 

Facilities

  • Maintains clean organized workspace and ensures confidentiality of client information.
  • Keeps vehicles and facilities clean.
  • Communicates any observed maintenance or safety issues immediately.

 

Documentation

  • Accurately documents all client meetings and discharge planning.
  • Accurately documents all referent and family communication.
  • Accurately documents team communication regarding referents and family.
  • Documents unusual or high-risk behavior and reports concerns to supervisor.

 

Primary Goals

  • Client transitions from RTC to Uinta 40-50% of RTC completions
  • Client transitions from RTC to VIOP = 20-25%+ of RTC completions
  • Total RTC completion transition goal = 65%+
  • Client transitions from Uinta to VIOP = 50%+
  • Client completion rates – all LOC – 80-85%+
  • Client satisfaction scores of 8 or higher in case management section


Qualifications:

  • Bachelor’s degree or higher preferred.
  • Must have a valid driver’s license in Utah.
  • Must be physically and mentally able to assist in an emergency situation.
  • Ability to interact in a clinical environment while maintaining appropriate boundaries.
  • Effectively communicate in a professional and knowledgeable manner.
  • Must have a TB screen within the last year or able to get one within 14 days of hire.
  • Current Basic Life Support and First Aid certification required, within 90 days of hire certification must be completed.
  • Previous experience serving individuals with mental health diagnoses, or substance use disorder is preferred.
