Position: Registered Nurse (RN) - Home Health Care


Location: Richmond Virginia

Responsibilities:

  • Conduct comprehensive assessments and develop individualized care plans.
  • Administer skilled nursing care in the comfort of clients' homes.
  • Monitor clients' health conditions and communicate with healthcare team members.
  • Educate clients and their families on health management and self-care techniques.
  • Provide compassionate support and companionship to clients.

Requirements:

  • Valid Registered Nurse (RN) license in the state of Virginia.
  • Minimum of 2 years of nursing experience, preferably in home health care.
  • Strong clinical assessment and critical thinking skills.
  • Excellent communication and interpersonal abilities.
  • Commitment to providing compassionate, client-centered care.
  • Age Friendly care.
  • CHAPS accreditation knowledge or experience is a plus.
