Job Description: Contracted PRN Nurse (Home Healthcare)

Position Overview: We are seeking a Contracted PRN Nurse to join our home healthcare team on a contractual basis. This role offers flexibility and autonomy, focusing on providing comprehensive intake assessments, discharge assessments, and maintaining meticulous charting. Key Responsibilities:

  1. Intake Assessments:

    • Conduct thorough initial assessments of new patients entering home healthcare services.
    • Collect detailed medical histories and assess current health status.
    • Collaborate with patients, families, and caregivers to develop personalized care plans.

  2. Discharge Assessments:

    • Perform comprehensive assessments prior to patient discharge from home healthcare services.
    • Evaluate treatment outcomes and patient progress towards goals.
    • Ensure patients and caregivers are educated on post-discharge care instructions.

  3. Timely Charting and Documentation:

    • Maintain accurate and up-to-date patient records using electronic health records (EHR) systems.
    • Document all assessments, interventions, and communications promptly and accurately.
    • Ensure compliance with regulatory standards and agency policies regarding documentation.

  4. Patient Care Coordination:

    • Coordinate care with interdisciplinary team members, including physicians, therapists, and social workers.
    • Communicate effectively with patients and their families regarding care plans and changes in condition.
    • Advocate for patients’ needs and ensure continuity of care during transitions.

Qualifications:

  • Registered Nurse (RN) with a current state license in good standing.
