Dynamic is seeking professional, reliable, and caring Home Health Vocational Nurse (LVN) to join our Medicare-certified Home Health Care Team We have an Immediate opening for a FULL-TIME/ PER DIEM LVN Home Health Nurse to provide Home Health visits to our patients across the Greater Los Angeles and Ventura Areas.

Responsibilities:

  • Implement plan of care initiated by the Registered Nurse
  • Provide accurate and timely documentation, consistent with the plan of care
  • Assess and provide patient and family/caregiver education, and information pertinent to diagnosis and plan of care
  • Use equipment and supplies effectively and efficiently
  • Be responsible for following all company policies and procedures
  • Perform other duties as assigned

Qualifications:

  • Previous experience in Home Health Nursing Care - One (1) Year
  • Ability to build rapport with patients
  • Strong problem solving and critical thinking skills
  • Ability to thrive in a fast-paced environment
  • Bilingual in Spanish a huge plus
