Location: Bronx, Duchess, and Portman County, NY

Job Description:

We are seeking experienced UAS Assessor Nurses to join our team. In this role, you will be responsible for conducting initial assessments for clients, determining their eligibility for services, and helping to develop comprehensive care plans tailored to their needs.

Key Responsibilities:

• Perform in-depth initial assessments using the Uniform Assessment System (UAS) to evaluate client needs.
• Document findings and recommendations in a clear and timely manner.
• Collaborate with healthcare professionals to ensure the best possible care for clients.
• Travel to client locations as required within the Bronx, Duchess, and Portman County.
