HumanEdge is recruiting for an experienced Clinical Resource RN. This is an amazing opportunity to join the team at a Specialty Pediatric Hospital.

  • Assess, plan, implement, and evaluate the nursing care for the residents while assisting with RN charge responsibilities
  • Building supporting relationships with the residents and families ensuring the liaison relationship supports the needs and rights of the residents.
  • Oversee accurate and thorough completion of the Minimum Data Set (MDS), care area assessments (CAA's), comprehensive care plans (CCP) and interim care plans, in accordance with federal and state regulations and guidelines that govern the process.
  • Working in collaboration with the interdisciplinary team (IDT) participate in regular care planning meetings.
  • Ensure all new admissions and readmissions are evaluated within 24 to 72 hours for Medicaid eligibility and /or significant changes in medical status and report such changes to the Director of Nursing.
  • Monitors the Case Mix Index scores and CASPER report looking for potential risks, while ensuring all regulatory reporting and guidelines are followed.
  • Bachelor of Science Degree in Nursing required.
  • NYS licensed RN.
  • A minimum of 1+ years MDS experience or 3+ years Long Term Care experience
  • Pediatric experience preferred
