Pride Health is hiring Wellness Coaches to work with a non-profit client in Brooklyn. The role is focused on a Housing Opportunities for Persons with AIDS (HOPWA) team to provide housing solutions for low-income individuals with HIV/AIDS and their families. Through multiple housing options, the Wellness Coach works with clients to learn about their situation/experience and help plan a path forward for their health and security.

Role: Wellness Coach

Location: Brooklyn, NY 11238

Overview: HOPWA housing options include permanent supportive housing, long-term rental assistance, and housing placement assistance. The goal of the program is to reduce homelessness, connect and retain clients in medical care, reduce HIV transmission, and improve adherence to antiretroviral therapies.

Pay: Contract-to-Permanent, $24.00/hour during contract period and $50k – 55k salary upon permanency

Schedule: Monday – Friday, 8:00a – 4:00p

Skills/Requirements:

· Bachelor’s Degree (required)

· Experience working with individuals with mental illness, substance abuse issues, HIV/AIDS, and/or a history of homelessness (preferred)

· Experience in group work, tenant organization, case management, referral, and/or community outreach strongly preferred.

· Experience or training in peer counseling or advocacy preferred.

· Previous supervisory experience preferred.

· Bilingual English/Spanish preferred.

Duties:

· Provide case management to assigned caseload and develop individual service plans.

· Assist clients with problem solving and crisis intervention; encourage and support self-advocacy.

· Identify problems in daily living skills; hands on assistance and/or referral in addressing issues with daily living skills.

· Educate clients on different housing options to determine the best option for their health and security.

· Supportive counseling to clients as needed, home visits on occasion/when necessary.

· Strong working relationship with all therapeutic support services/staff to ensure best quality of care.

· Documentation of all services in compliance with agency standards.

· Liaison to community-based services and groups (civic, social service, government, and business organizations)

· Participate in team meetings, case conferences, division meetings, and trainings.

· Adhere to on call cell phone to facilitate crisis intervention, emergencies, and other situations requiring immediate action.
