Job Title: PACT Youth & Family Coordinator (YFC)

Reports To: PACT Supervisor

Work Environment: 25% in Office

Our mission is to coordinate resources, information, and a network of quality providers. We are currently seeking a PACT Youth & Family Coordinator, who will play a pivotal role with youth to improve their success and sustainability in a community environment. PACT-Permanency focused, Action oriented, Collaboration, Team-based is the coordination program for young people experiencing foster care, receiving adoption assistance, or moving to permanency with an appropriate resource who may have significant mental health challenges.

Job Purpose:

The PACT YFC is responsible for coordinating and facilitating the entire support process for each of the youth/families served by maintaining accurate records of meetings, timely requisitions and reporting of service requests, resources, and ongoing communication with all team members. This role is crucial in supporting the implementation of timely individualized goal-oriented action plans for youth and useful safety plans for crisis situations. The PACT YFC is responsible for ensuring youth and families served have every opportunity to be successful in the home, school, and community settings.

Key Responsibilities:

  • Coordinate ongoing meetings with youth, families, partner agencies and community members such as DFCS, DJJ, and school systems to identify strengths & explore needs of the team, youth and families served.
  • Organize and prepare for meetings including but not limited to, drafting, and developing action and safety plans, identifying, and developing individualized resources to meet the unique needs of the youth and families served.  
  • Statewide travel to agencies, foster homes, and birth homes to meet and interact with youth and families.
  • Entering documentation within required timelines into Exponent Case Management (ECM) 
  • Attend and participate in all relevant meetings pertaining to PACT youth and program.

Qualifications:

Essential:

  • High school Diploma required, Bachelor’s degree required in Human Services (preferred) or related field.
  • Excellent verbal and written communication skills
  • Excellent organizational skills and attention to detail.
  • Excellent time management skills with proven ability to meet deadlines.
  • Proficiency in Microsoft 365 Suite, or similar software with ability to learn new or updated software.
  • Strong strategic, analytical, and problem-solving skills.
  • 2 years of relevant experience in coordination of resources and services for youth and families.
  • Ability to function well in a high-paced and at times stressful environment.

Preferred:

  • Knowledge of the Georgia Child Welfare, Behavioral Health System, Educational system, and MAAC and its many functions.
  • Experience working with and in a youth or family-centered practice model and crisis intervention.

Work Environment:

This role is a full-time, 40-hour a week position that generally works in the field and requires occasional monthly in-office presence in Downtown Atlanta. To effectively support the youth, the expected work week is Monday through Friday with some evenings and weekend work as needed.
