Inpatient Clinical Pharmacist

RPh on the Go is actively interviewing candidates for a Clinical Pharmacist position in Wilmington, Delaware. As a Clinical Pharmacist, you will play a crucial role in ensuring the safe and effective use of medications for our patients. You will collaborate with healthcare providers to optimize medication therapy and contribute to the overall well-being of patients. Responsibilities:
Medication Therapy Management:
- Conduct comprehensive medication reviews for patients to identify and resolve medication-related issues.
- Collaborate with healthcare providers to develop and implement patient-specific treatment plans.
Clinical Support:
- Participate in interdisciplinary rounds and collaborate with physicians, nurses, and other healthcare professionals to optimize patient care.
- Provide drug information to healthcare providers and assist in the development of evidence-based guidelines.
Quality Improvement:
- Contribute to quality improvement initiatives by monitoring medication use processes and implementing improvements as needed.
Regulatory Compliance:
- Ensure compliance with state and federal regulations related to pharmacy practice.
- Stay current with new medications, treatment guidelines, and emerging trends in clinical pharmacy practice.
Qualifications:
- Clear and active pharmacist license in the state of Delaware
- Previous Inpatient Pharmacy experience preferred
