Schedule:
- Flexible (3, 4, or 5-day work week)
- Full Time (30-40 hours weekly)
- Day shift only
- No weekends
- Fixed schedule/no on-call
Key Responsibilities:
- Must be able to function at a high level with little oversight to provide nursing care, training, and home support (as needed);
- Works collaboratively with Dialyze Direct team members, skilled nursing home facility representatives and dedicated Nephrologists;
- Collects and records relevant patient health data in the course of/pre/post hemodialysis treatment;
- Develops and maintains a plan of care that describes interventions to attain expected outcomes;
- Implements and records interventions to attain expected outcomes in EMR;
- Performs laboratory collection, processing, and shipping procedures and reschedules missed or inadequate laboratory collections;
- Perform clerical tasks in addition to patient care (i.e., filing, inventory management, admission documentation, creating patient charts, etc.);
- Maintains a positive and respectful attitude to all patients and colleagues;
- Occasional travel to other nearby locations; and
- Medication administration and documentation.
Requirements:
- BS/AA/diploma in nursing with at least 12 months of experience (3 months in hemodialysis) preferred;
- Valid RN license;
- Valid drivers license and transportation
- Ability to travel covering multiple locations
- BLS certification; and
- Ability to lift up to 35 pounds is required.

Compensation:
Salary Range: $32.00 - $55.00 per hour. **BASED ON EXPERIENCE.**
