ESSENTIAL FUNCTIONS:

  • Conduct a comprehensive assessment of client health and psychosocial needs, including health literacy status and deficits.
  • Develop a case management plan collaboratively with the client, family or caregiver.
  • Coordinate the care with the primary care provider, other health care providers, the payer and the community, to maximize health care responses, quality and cost effective outcomes.
  • Inform the client, family or caregiver and members of the health care delivery team about treatment options, community resources, insurance benefits, psychosocial concerns, case management, etc., so that timely and informed decisions can be made.
  • Empower the client to problem-solve by exploring options of care as available and alternative plans when necessary to achieve desired outcomes.
  • Strive to promote client self-advocacy and self-determination. Advocate for both the client and the payer to facilitate positive outcomes.

OTHER FUNCTIONS:

  • Perform other functions and tasks as assigned.

EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:

  • Bachelor’s degree in social work, Counseling or related human service field or three years’ experience working in behavioral health setting working with the patient population of the facility.
  • Demonstrated experience utilizing clinical intervention skills.
  • Minimum of two years experience with mental health clients

LICENSES/DESIGNATIONS/CERTIFICATIONS:

  • Licensure or certification for counseling, as may be required by the state in which the facility operates. First-Aid, CPR and de-escalation/restraint certification required (training available upon hire and offered by facility).
  • NPI number required
