  • Adheres to all company, policy and procedures.
  • Responsible for pulling daily patient schedule for presentation in morning huddle to communicate other team members items that may need to be addressed for patients that will be seen at the center.
  • Actively participate in the daily team huddle.
  • Review daily patient schedule to determine need for implementation of standing orders/preventive reminders. This process should start 2-3 days prior to the patients scheduled appointment date.
  • Check medical records and obtains any pending results prior to patient’s appointment.
  • Assist with daily patient flow.
  • Greets patients courteously addressing them as Mr. or Mrs., etc and by their last name unless patients request that they be referred to by their first name.
  • Takes and records vital signs, weigh patients, conducts COA questions and documents these accordingly in the patient medical record.
  • Reviews patient medications with patients and documents that such was conducted at the time of service.
  • Ensures that all chronic care medications are dispensed as 90 day supplies, unless otherwise indicated by provider.
  • Safely escorts patients in the facility to the exam rooms.
  • Recognizes abnormal vital signs and reports these immediately to physician.
  • Performs all procedures ordered by the physician.
  • Responsible for verifying that all test ordered were performed before the patient leaves.
  • Perform EKG and give physician the results as well as scans these in the patient electronic medical record on the same date of service.
  • Assists in achieving performance measures (HEDIS) by identifying need for evidenced-based preventive care.
  • Identifies patients that may meet guidelines as vulnerable patients and suggests forwarding to case management services to physician.
  • Use effective communication skills to assist patient in self-management and behavior change.
  • Communicates to provider any outstanding care gap such as mammograms, colonoscopy, etc.
  • Promotes and reinforces patient centered medical home concepts with patients and staff.
  • Performs lab work as per physician orders.
  • Prepares instrumental equipment for sterilization if needed.
  • Assists physician with examination as required and assists other staff members in providing patient care.
  • Communicates observations of patient’s status to physician.
  • Complete all patient information on all forms given by physician and instruct patient accordingly.
  • Discharges patients as indicated by the physician.
  • Remains in work area until the physician has seen all patients.
  • Responsible for communicating to the office manager the need of medical supplies according to the department needs prior to running out.
  • Checks equipment and supplies prior to use for function, inspection, and expiration date.
  • Reports malfunction of medical equipment and safety issues to supervisor.
  • Checks refrigerator temperature and reports abnormal readings to providers.
  • Complies with Infection Control and OSHA Regulations (biomedical waste, medication handling, and disposal) for the practice as per regulations.
  • Complies with HIPAA laws.
  • Obtain specimens in appropriate containers, labels correctly and processes requisitions with appropriate lab (contracted lab of patient’s insurance carrier).
  • Maintain log for different procedures as required.
  • Call lab for specimen pick up and place in appropriate lab box outside.
  • Administers medication following the seven rights of medication administration. Right patient, medication, dose, route, time, reason and documentation.
  • Assist providers in completing AWV visit questions.
  • Phones in prescriptions as ordered by physician.
  • Obtains referral authorizations as needed and schedules patient appointments accordingly.
  • Keep exam rooms, lab and x-ray area clean at all times.
  • All exam tables must be disinfected between patients and at the end of a work day.
  • Stores drug samples and removes any expired medication.
  • Documents dispensing of any sample medication.
  • Takes appropriate action to maintain a safe environment.
  • Ability to answer phones, schedule/reschedule appointments, and transportation.
  • Ability to check-in / check-out patients in a professional and timely manner.
  • Ability to verify patient insurance eligibility and report any policy cancelations or erroneous PCP assignations to the patient center manager.
  • Ability to obtain a referral authorization for all plan types as well as schedule diagnostic, specialist, and surgical procedures with or without transportation.
  • Ability to confirm appointments and schedule/reschedule as needed.
  • Assist patients and providers in connecting remotely to facilitate Telehealth Services.
  • Train patients and/or other team members on the use of Telehealth Services at either the center and/or patient's place of residence.
  • Available and flexible with work schedule and location, to include weekends as needed.
  • Ability to conduct medical assistant duties as ordered by PCP at the center and/or patient's place of residence.
  • Able to demonstrate the proper use of Personal Protective Equipment.
  • Able to follow all safety and universal precautions.
  • Must have a clean and active driver's license as well as provide proof of car insurance, should travel be required.
  • Perform other duties as assigned.
