Schedule: Approximately 8 hours a week. Two days a week at 4hrs/day. This is a Contractor (1099) position.
Job Responsibilities:
- Conduct an initial nutrition assessment for new participants and perform reassessments every six months and more frequently as participants needs change.
- Develop and implement a nutritional Plan of Care with recommendations. Reevaluate Plans of Care at reassessment intervals and/or when a participant’s condition changes.
- Provide counseling and education of the dietary needs of participants.
- Analyzes menus to meet all applicable regulations of the CACFP (child and adult care food program). **Knowledge of CACFP is required.
- Provides guidance to the Food Service Manager in creating menus that are changed monthly.
- Participates in developing and monitoring policies and procedures as they relate to food service production and service.
- Provide education and training to program staff.

Psycho-social services, such as counseling and social work
