Job Summary

The role of the Inpatient Care Management Nurse is to coordinate continuity of care for patients and, as necessary, act as a liaison between the patient's family, provider and healthcare organization, supporting the proper course of treatment at the appropriate time to maximize health and well-being. The individual in this position will be responsible for connecting primary care groups to their patients within the inpatient and emergency room settings. The primary focus will be facilitating the seamless transition of care from the hospital to home or a Post Acute setting. This position integrates care coordination, utilization management, and discharge planning.

Role and Responsibilities

  • Act as a liaison between primary care groups and the inpatient care team to ensure efficient communication and coordination.
  • Facilitate the timely and effective transition of patients from the hospital to their homes or Post Acute settings, promoting a smooth continuum of care.
  • Provide ongoing updates to PCPs regarding patient status and any changes in treatment plans or care needs.
  • Schedule outpatient testing and follow-up appointments required for continued patient care.
  • Monitors the care plan to determine if the goals are being met on an ongoing basis to evaluate for needed changes and updates the plan of care accordingly.
  • Collaborate with emergency room (ER) doctors and staff to ensure patients receive adequate PCP follow-up before being discharged.
  • Advocate for patients and address any barriers to effective care coordination, such as medication reconciliation, access to healthcare services, or post-discharge support.
  • Participate in interdisciplinary meetings and discussions to ensure comprehensive and patient-centered care plans are in place.
  • Document all care management activities accurately and thoroughly.
  • Communicates and collaborates with Medical and Nursing staff.
  • Promotes the mission, philosophy, goals, and policies of the organization through staff education.
  • Completes clear and concise documentation in Care Management programs.
  • Participates in the quality care conference program.
  • Provides clinical oversight of the care plan and care coordination process implemented by Care Coordinator.
  • Maintain personal professional development.
  • Collaborates with providers and/or the Medical Director when appropriate (i.e., communicates review findings, criteria not met, use of alternative care settings, determination of appropriate level of care, delay of provision of services, etc.) to insure appropriate, coordinated service delivery.
  • Participates in departmental QIP process
  • Performs other duties as assigned

Qualifications and Requirements

  • Must possess a current and active RN license to practice in the state(s) assigned or maintain a compact license.
  • 3+ years of clinical experience with a strong clinical background and understanding of patient care processes in both inpatient and emergency room settings.
  • Excellent communication skills, both written and verbal, with the ability to effectively interact with patients, physicians, and other healthcare professionals.
  • Proven ability to organize and prioritize tasks, manage multiple responsibilities simultaneously, and meet deadlines.
  • Demonstrated knowledge of care coordination principles, transitional care, and discharge planning.
  • Proficiency in using electronic health record (EHR) systems and other relevant software applications.
  • Accountable and autonomous.
  • Ability to handle multiple demands of diverse workload and prioritize critical issues.
  • Ability to influence and effect change.
  • Ability to analyze and think critically.
  • Possesses current knowledge of disease pathophysiology, psychosocial issues, and treatment.
  • Good time management skills.
  • Positive, service-oriented attitude.
  • High level of integrity.
  • Ability to maintain a HIPAA compliant professional work environment.
  • CCM highly desirable
