SUMMARY:

Under the general supervision of the Director of the specified Division or Program, and in cooperation with the Department Clinical Operations Manager and site Practice Manager, is responsible for coordinating patient care activities for selected patients. "Clinical navigation" refers to the assistance offered to healthcare consumers (patients, survivors, families, and caregivers) to help them access and then chart a course through the healthcare system and overcome any barriers to quality care. The clinical navigator functions as the guide who will assist patients and their families in moving through the complexities of the healthcare system. Travel is required between our Providence, East Providence, and South County sites.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

  • Management of surgeon pathology logs including logging excisions, biopsies and wound cultures into the pathology log, calling patients with results and coordinating the correct care
  • Management of case follow up log – including coordinating and referring patients to MDC, radiation, and/or plastics. Following up on cases and finalizing all treatment plans.
  • May be required to assist in resident cosmetic clinic.
  • Availability to assist in the management of cosmetic patients when needed (ex. scheduling, cosmetic call backs, and scrubbing cosmetic schedule).

BASIC KNOWLEDGE:

  • Bachelor’s degree in related discipline (preferred).
  • Proficiency in pathology log management required
  • 1+ years Mohs experience required
  • Professional and analytical skills in evaluating patients in order to plan and provide patient care and monitor the care management process and treatment plan for each patient.
  • Ability to communicate effectively (public speaking skills) with diverse audiences including professional staff, patients, community, media, and other service agencies.
  • Interpersonal skills to allay anxiety, elicit cooperation, and provide for education of patients/families, and to effectively function as a member of the healthcare team to coordinate and discuss patient care activities.
  • Advanced skill in assessment of physical and psychosocial health-illness status of individuals, history taking, and physical assessment.
  • Demonstrated knowledge and skills necessary to provide care to patients with consideration of aging processes, human development stages, and cultural patterns in each step of the care process.

EXPERIENCE:

Medical Assistant with 2+ years of Dermatology experience, preferably in an academic setting.
