Compensation: $125 per patient visit

Who We're Seeking: We are seeking a compassionate and skilled Hospice Nurse Practitioner to join our team on a per diem basis. The ideal candidate will have a deep understanding of hospice care, excellent clinical skills, and the ability to provide empathetic support to patients and their families. This role involves working closely with our interdisciplinary team to manage pain and symptoms, and to ensure the highest quality care.

Your Responsibilities: Visit 2-3 Hospice patients in their homes weekly and provide additional support as needed.

  • Conducts face-to-face encounters for patients in their third or subsequent benefit period, documenting and sharing findings with certifying physician promptly.
  • Coordinates and communicates with IDG members, consulting physicians, facility staff, and others as needed.
  • Participates in developing Plan of Care for assigned patients.
  • Assists in enhancing care strategies with Hospice Clinical Manager and Medical Director.
  • Acts as a resource for Hospice staff to ensure continuity and improve patient care quality.
  • Attends IDT meetings as requested by Hospice Clinical Manager.
  • Provides backup support to Hospice Medical Director, covering medical needs of hospice patients as required.

Requirements:

  • Graduate of an approved Nurse Practitioner program
  • Active Vermont APRN or NP license required
  • Minimum of two years clinical nursing experience, hospice preferred
  • Valid driver’s license
