JOB SUMMARY

The Nurse Practitioner is responsible for conducting a face to face encounter and documenting the clinical findings supporting a life expectancy of 6 months or less. ***This is a PRN position, paid per visit ($100/visit)***

MAJOR JOB FUNCTIONS
  • Promote and practice the Sage Palliative philosophy and mission: To provide the most compassionate, comprehensive, and qualified care to people with a life-limiting illness to serve them through their individual journey with dignity, respect, and integrity.
  • Working knowledge of the regulatory requirements per state licensure, federal guidelines
  • Document patient F2F encounters in a timely manner immediately following the encounter
  • Demonstrate an understanding of the CMS regulatory requirements specific to documentation as it relates to the patients F2F encounter, and physical and functional decline
  • Properly utilizes prognostication tools such as, but not limited to, PPS, FAST scores, ESAS,NYHA
  • Documents utilizing quantifiable and comparative data from a full patient assessment • Advocate for healthcare changes which have the potential to improve quality of life and identify grief issues of patients and families by timely reporting of any concerns to a hospice clinical manager
  • Collaborate with health care partners and referral sources to promote and sustain community relationships
  • Maintains proper certifications and licensing per state guidelines
  • Proficient in accessing and utilizing the organization’s EMR
  • Perform all other duties as assigned
REQUIREMENTS
  • Master’s degree in nursing from an accredited program with current license to practice professional nursing in the state in good standing
  • Board-certified advanced nurse practitioner
  • Current DEA
  • CPR certification
  • Minimum of 2 years of health care experience in hospital, medical outpatient clinic, home health, hospice, or specialty care setting
  • Must be a licensed driver with an automobile that is in good working order and demonstrate ongoing insurance in accordance with state and or organization requirements
