Position Summary:
Intake Coordinator responsibilities include answering high call volumes and making calls, collecting and entering data in databases and maintaining accurate records of valuable company information with an eye for detail and familiarity with spreadsheets and online forms. The coordinator speaks directly with the person and their families to determine what services might be needed and then guide them to the right area. You will work with an intake team and intake manager

ESSENTIAL DUTIES:
  • Answering and making phone calls
  • Speaking with prospective clients and utilizing assessments to determine resources to meet the needs identified on the needs assessment.
  • Diligently track all referrals, forms, and outcomes as it relates to services for the client.
  • Work collaboratively with the Sales Team, Human Resources and Respect and Care Center.
  • Maintain a positive attitude toward all clients and their families 100% of the time.
  • Responsible for inputting potential clients information and assessments into Caryfy.
  • Communicates regularly with potential clients throughout the intake process.
  • Coordinates timely follow-up with potential clients and their families through phone contact when needed.
  • Entering clients data by inputting text based and numerical information from source documents within the time limits.
  • Compile, verify accuracy and sort information according to priorities to prepare source data computer entry.
  • Other duties as assigned
REQUIRED KNOWLEDGE, SKILLS, ABILITIES AND COMPETENCES:
  • Excellent written and verbal communication skills.
  • Excellent organizational and time management skills
  • Excellent communication, interpersonal, and conflict resolution skills.
  • Demonstrate teamwork, initiative, and willingness to learn
  • Ability to exercise discretion and professionalism when handling highly confidential and sensitive information.
  • Data entry with an exceptional typing speed.
  • Requires basic knowledge of Microsoft Windows and Microsoft Outlook.
MINIMUM QUALIFICATIONS:
  • High School diploma/GED with a minimum
  • 3 + years of experience working in customer service
  • Must have knowledge of senior community and area resources.
