The Medical Case Manager plays a pivotal non-clinical role within the Homeless Resource Centers support team, serving as a compassionate advocate for patients in need. Working primarily at the HRC's, they collaborate closely with the healthcare team to identify vulnerable patients and conduct thorough needs assessments. With a patient-centered approach, they create and maintain care plans tailored to individual needs, coordinating medical, behavioral health, and support services both within the clinic and with community partners.
