Primary Care HIV Prevention (PCHP) is a U.S. Health Resources and Services Administration funded program aimed at decreasing the risk of HIV transmission, focusing on supporting access to and use of pre-exposure prophylaxis (PrEP). The PCHP program focuses on increasing diagnosis, treatment, and prevention of HIV at JPA.

The PCHP PrEP Navigator is responsible for coordinating care for patients including patients at high-risk for HIV and patients prescribed PrEP; arranging linkages to care for patients; and establishing professional linkages with identified HIV/AIDS and health and social support services providers in the targeted project communities. The PrEP Navigator will help develop policies and programs to increase HIV testing, prevention, and PrEP services at JPA.


ESSENTIAL DUTIES AND RESPONSIBILITIES:

1. Provide culturally competent education on HIV prevention, PrEP, PEP, and available social services to JPA patients

2. Perform light case management for patients by providing appointment reminders and assisting patients prescribed PrEP and PEP in developing and adhering to a medication management schedule

3. Coordinate linkages to care for clients seeking treatment adherence support, housing, entitlements, ham reduction education, and health insurance

4. Ensure appropriate follow-up for client referrals to external resources such as substance use treatment, mental health services, restorative justice, legal services, and nutrition support

5. Conduct sexual and social history screenings for linkages to appropriate care/services

6. Reduce barriers to obtaining clinical/care services by working with providers and other clinic staff to ensure that HIV care is treated as primary care

7. Prepares and presents appropriate educational and other materials, using harm reduction approaches and evidence-based information

8. Support PCHP Health Educators with planning and coordinating educational outreach events including those targeted to at-risk communities

9. Help identify and address HIV/HCV testing, linkage, and retention service gaps

10. Foster relationships with community partners to streamline referrals and linkages to social services

11. Work closely with Care Teams across the JPA network to provide high quality comprehensive care to patients throughout Addabbo clinics

12. Maintain accurate and updated patient records, tracking contacts, counseling, testing, referrals, and outreach performance as defined by evidence-based models, local/state/federal entities, and JPA

13. Participate in staff meetings and staff development trainings

14. Perform special projects and other job-related duties as assigned


MINIMUM QUALIFICATIONS:

1. Associate/Bachelor’s Degree in Health Education, Public Health, Social Work, or related field preferred. In lieu of degree, two years minimum work or volunteer experience in field with patient contact

2. Members of the LGBTQIA+ community and people living with HIV/AIDS are strongly encouraged to apply

3. 2 years case management experience preferred

4. Experience working with PLWHA, LGBTQIA+, and communities of color preferred

5. Computer literacy with proficiency in MS Word, Excel, and PowerPoint as well with electronic medical records preferred

6. Demonstrated organizational, interpersonal, oral, and written communication skills and the ability to handle multiple assignments at any time

7. Comfort with working independently and collaboratively in a remote environment

8. Must have cultural competency and comfort working with LGBTQ populations and communities of color

9. Bilingual (English/Spanish) preferred.
