JOB DESCRIPTION

JOB TITLE: LEP Case Worker DEPT: Supportive Services

REPORTS TO: Program Coordinator FLSA: Non-exempt

SALARY RANGE: STARTING SALARY:

POSITION SUMMARY: Works with all Alliance clients/families who have limited English proficiency to identify barriers and gaps in services, and tracks outcomes to determine how customers are doing as a result of the services provided.

ESSENTIAL FUNCTIONS:

  1. Interview and enroll clients/families, who have limited English proficiency, in our various programs based on their needs and their income eligibility. Case Management has no income limit.

  1. Assist the family in its own efforts to improve conditions, receive services they need and are eligible for, and promote self-sufficiency through action plans.

  1. Work with client/family to develop a personal action plan that identifies individual/family goals and helps them achieve economic independence and self-sufficiency.

  1. Referral and follow-up activity with community resources in response to health and social service needs of clients/families.

  1. Follow up with clients at least every 90 days.

  1. Engage in advocacy work on behalf of individuals/families.

  1. Write and Maintain extensive Case Notes on the status and progress of each client.

  1. Able to learn and use CAPTAIN's systems case management & other software required by funding sources proficiently.

  1. Participate with an interdisciplinary team in regular case conference meetings to mobilize and coordinate services within and outside the agency on behalf of assigned clients/families.

  1. Be aware of Social Services and Health agencies and develop a working relationship with staff persons for a smoother transition when making referrals.

  1. Establish and coordinate a community resource data bank for families.

  1. Prepare required reports.

  1. Enter data into a computer.

  1. Performs other work-related duties as assigned.

EDUCATION, EXPERIENCE AND SKILLS REQUIRED:

  1. Bachelor’s degree in Human Services or related field plus at least one year experience in Human Services. OR Associate degree in Human Services or related field plus at least three years experience in Human Services. OR a High School graduate with five years experience in Human Services.

  1. Knowledge of Social Service/Health Agencies in the Greater Bridgeport area.

  1. Bi-lingual oral and written communication skills are required. (Spanish, Creole, Portuguese)

  1. Strong verbal and interpersonal skills are a must.

  1. Computer proficiency is required.
