Job Summary:

As a Social Services Coordinator, you will be responsible for providing emotional support, counseling, and resources to our residents and their families. You will work closely with our interdisciplinary team to ensure that our residents receive the highest quality of care.

Job duties:

The below statements are intended to describe the general nature and scope of work being performed by this position.   • Visits resident(s) upon admission to assess the social, emotional and environmental needs of the resident(s) and monitors on an ongoing basis.
  • Prepares and maintains required documentation/assessments as regulated by local, state and federal agencies and as problems/situations occur.
  • Coordinates and participates in ongoing evaluation of the residents' needs and goals through resident care plan meetings.
  • Monitors adherence to plan of care and flow of information. Provides continuing contact with resident/family to coordinate ongoing care. Provides crisis intervention, reassessment and uncovers problems as they occur in the delivery of services.
  • Provides counseling and emotional support in relation to health/social needs and plan of care. Provides ongoing emotional support to residents and families to enable them to cope with difficulties and proper utilization of services.
  • Acts as liaison between resident/family and relevant professionals, programs, agencies, etc. to help resident secure services needed and make preferences known.
  • Coordinates discharge planning. Participates in internal transfers throughout the center as the need arises.
  • Coordinates the placement of residents to alternative care facilities when necessary.
  • Acts as an advocate for residents' rights and needs.
  • Collaborates with the interdisciplinary team to ensure that residents receive comprehensive care.
  • All other duties assigned


Requirements:


  • Bachelor's Degree in Social work or related field preferred but not required
  • One or two years previous experience in medical setting required, nursing home/LTC experience preferred
  • Skills needed in group process and communications, both verbally and written
  • Basic skills in Microsoft Office Suite of products
  • Ability to interact with other professionals in a teamwork manner
