Cook Hospital & Care Center is hiring a Full Time Nurse Manager - Care Center in our Long-Term Care facility. In this role, the Registered Nurse (RN), Nurse Manager is a professional caregiver who assumes responsibility and accountability for resident for a designated timeframe and provides care to these residents via therapeutic use of self, the nursing process, the environment/instrumentation, and other health care team members. Demonstrate an overall understanding and commitment to the facility’s mission and philosophy. Assumes responsibility for supervision of the Care Center. Must perform professional assessments and coordination of care needed for the residents. Provides supervision and direction to nursing staff. Develops and promotes cooperative working relationships in nursing services as well as between all departments as they relate to the Nursing Home. Works collaboratively with the MDS/PPS coordinator to complete assessments on residents per MDS schedule and collaborates with the DON with all daily activities.

Qualifications:

          Licensure/Certification/Registration: Current RN licensure by Minnesota State

Board of Nursing is required.                                           

Education: Graduated from an accredited school of nursing (RN).  BSN preferred.

Experience:  Minimum of 2 years nursing experience in long term care with supervisor-manager experience preferred.

Essential Functions:

  • Administer medications and operate medication cart if needed.
  • Start IV and give IM medications, hang IV piggybacks if needed.
  • Complete documentation including Meditech charting on computer and patient charting in hard copy form.
  • May provide training to RNs, LPNs, and NAs
  • Complete quality assurance tasks
  • Deliver care to residents to meet their needs.
  • Provide various treatments ordered by the physician including dressing changes to wounds and skin care, etc.
  • Assist NAs with passing meals and feeding residents.
  • Perform CPR and/or First Aide when necessary.
  • Oversee care delivered by RNs, LPNs, and NAs
  • Complete assignments within a reasonable amount of time
  • Communicate effectively orally as well as written.
  • Attends required in-services.
  • Perform, coordinate, supervise and develop nursing activities of the Care Center
  • Plans and organizes activities on a station to ensure resident needs are met in accordance with instruction of physician, Care Center policies and procedures and Federal, State, and local regulations.
  • Perform cares and supervise care given by nursing personnel and assume responsibility for nursing care delivered to the residents by nursing staff.
  • Makes routine rounds to assure that nursing service personnel are performing required duties and to assure that appropriate nursing care is being delivered to the residents which meet the needs of the residents.
  • Assesses residents’ condition and ensures that changes in condition and appropriate information are reported to the Physician, family, and appropriate facility staff. 
  • Monitors to ensure that physician’s orders are appropriately followed.
  • Communicates and help develop communication tools that direct the activities of each shift to appropriate persons through written and oral reports/recommendations as required.
  • Follow-up on lab work according to facility policy and results are documented in the chart and communicated to the physician.
  • Provides direct care and supervision to nursing employees and monitors nursing care given.
  • Administers professional services such as catheterizations, tube feedings, suctioning, applying/changing dressings and bandages, colostomy, and drainage bags, draws blood as required, works with IV medications, etc.  Teaches and oversees nursing staff to function in these areas.
  • Ensures documentation is accurate and complete.
  • Complete MD Round sheets prior to MD rounds.
  • Monitors the administration of medications as ordered by the physician within Federal, State, and local regulations and required by facility policy.
  • Orders prescribed medications, supplies, and equipment as necessary within established facility policy.
  • Monitors the use of narcotic medications according to Federal, State, and local regulations as per facility policy.  Ensures that narcotic records are accurate for per facility policy.
  • Will assist DON with performance appraisals for nursing staff assigned to unit at least annually.
  • Maintain harmonious relationship among nursing personnel and participate in conflict management.
  • Must work as part of the interdisciplinary team with the Director of Nursing, MDS/PPS coordinator, and Administration. Keeps the DON and administration informed of pertinent information.
  • Participates as needed with DON and HR in handling staff counseling, discipline, promotion, or termination.
  • May oversee work schedules when necessary and workers in order to maintain coverage of the Care Center.
  • Must be flexible in response to unexpected changes in workload, emergencies and or scheduling changes.
  • Coordinate and help maintain Resident Care Plans.
  • Works specifically on quality projects including QAPI, Psychotropic and Pain Committees, IDT, and other committees as assigned by the DON.
  • Develops and maintains a good working rapport with interdepartmental personnel, as well as other departments within the facility to assure that the needs of the residents are adequately met.
  • Consults with the Director of Nursing-on-nursing problems and interpretation of Care Center policies to ensure resident needs are met.
  • Assists in developing, implementing, and coordinating department policies and procedures, resident/nursing care plans, nursing objectives and standards.
  • Monitors for appropriate notification to physician of resident assessments and/or change in the medical care plan.
  • Makes rounds with physicians as necessary.
  • Ensures the appropriate delivery of the residents’ plan of care.  Monitors the accurate and safe delivery of all physicians’ orders.
  • Practices Standard Precautions at all times and assists infection control coordinator/Director of Environmental Safety in training & auditing staff for compliance with the infection control plan for the facility.
  • Attends mandatory in-services offered by the facility.
  • Follows facility safety policies and procedures.
  • Coordinate the completion of MDS/PPS assessments, care plans and written or physical MDS assessments/interviews as needed or directed by MDS/PPS coordinator or in absence of MDS coordinator.
  • Perform other essential functions of the MDS/PPS position as needed in absence of MDS/PPS coordinator.
  • Attends and prepares nursing overview for individual resident care conferences.
  • Provides follow up and documentation on skin, falls and behavior committees. Participates and/or facilitates other committees as assigned or necessary.
  • Will complete other duties as assigned.
  • Prepares and assists with Physician Rounds
  • Involved in admission and discharge process of residents.
  • Other duties as assigned by the DON.
