Position Summary:

The Care Coach will help ensure that our clients receive safe, professional, and loving care in their homes. The Care Coach will be a vital part of our operation by providing onboarding and training to our caregivers to ensure they are providing the best quality of service to their loved ones. This position will support Cluster G (Tipton, Hamilton, Marion, Hendricks, Morgan and Johnson counties).

Key Responsibilities:

  • Home orientations involving signing of paperwork.
  • Periodic supervisory visits
  • Training caregivers/ patients as required per case.
  • Interacting with the Care Manager for the patient as needed.
  • Engaging in interdisciplinary meetings with Care Managers, Caregivers and patients.
  • Travel within the assigned territory to see patients and clients as needed in their homes.
  • Document patient information accurately and maintain records.
  • Perform other duties as assigned.

  • Maintain confidentiality as related to client information under HIPAA regulations regarding Protected Health Information. Any disclosures of confidential information made unlawfully outside the proper course of duty will be treated as a serious disciplinary offense.


Education:

  • High school diploma or equivalent

Qualifications:

  • A minimum of 2 years’ hands-on experience in home care.
  • Must be detail oriented.
  • Experience working with medically complex populations.
  • Demonstrate good organizational skills and time management skills.
  • Must have the ability to travel on a weekly basis as this role is a field-based position.
  • Must have an active driver license and reliable transportation to drive throughout the day / territory.
  • Ability to effectively communicate with patients, families, and healthcare professionals.
  • Strong critical thinking and problem-solving skills.
  • US work authorization (Required)
  • Must have your own computer and high-speed internet.

Job Type: Full-time

Salary: $50 per visit, plus mileage

Schedule:

  • Monday – Friday; day shift
