Speech Therapist Opportunity in Peninsula - Home Health Setting

Role Summary: We are seeking a compassionate and skilled Speech Therapist to provide exceptional care to patients in their homes across the Peninsula area. This position is ideal for someone passionate about helping patients improve their communication abilities and swallowing functions, ensuring they achieve the best possible quality of life.

Responsibilities:

• Assess, diagnose, and treat speech, language, social communication, cognitive-communication, and swallowing disorders in patients across all ages.
• Develop and implement individualized care plans that cater to patient needs and goals, in collaboration with our interdisciplinary team.
• Monitor and document patient progress, adjusting treatment plans as necessary to ensure optimal outcomes.
• Provide guidance and education to patients, families, and caregivers to support patient progress and independence.
• Ensure timely and accurate documentation of therapy sessions and patient progress using the provided electronic health record system.
• Adhere to all professional, ethical, and company policies and procedures.

Qualifications:

• A current California license as a Speech Therapist.
• Valid CA Driver’s License and the ability to travel within the designated service area.
• Experience in a home health setting is preferred but not required.
• Strong interpersonal and communication skills, with the ability to work effectively with patients, families, and an interdisciplinary team.
• Proficiency with electronic health record systems, with the ability to complete documentation accurately and promptly.
