Family Nurse Practitioner

The Family Nurse Practitioner (FNP) will play a crucial role in providing comprehensive and patient-centered healthcare services to individuals and families within a diverse suburban and rural community in Southeast Texas. Operating within a community health center setting, the FNP will collaborate with a multidisciplinary team to deliver high-quality primary care, preventive services, health education, and disease management to patients of all ages.
- Patient Assessment: Conduct thorough assessments of patients' medical histories, perform physical examinations, and order relevant diagnostic tests to develop accurate diagnoses and treatment plans.
- Diagnosis and Treatment: Provide evidence-based medical care to patients, including prescribing medications, recommending treatments, and developing care plans that address acute and chronic health conditions.
- Primary Care: Offer primary care services, focusing on health promotion, disease prevention, and health maintenance. Provide routine check-ups, vaccinations, and screening. Refers patient to appropriate licensed physicians, specialists, hospitals, or emergency rooms as needed.
- Chronic Disease Management: Manage and monitor patients with chronic illnesses such as diabetes, hypertension, and asthma, offering education, lifestyle counseling, and medication management to optimize health outcomes.
- Collaborative Care: Work collaboratively with physicians, nurses, social workers, and other healthcare professionals to ensure comprehensive care and patient referrals for specialized services, when necessary, utilizes agency resources such as care managers to assist with obtaining community and medical resources to meet the needs of patients.
- Health Education: Educate patients and families about health-related topics, including disease management, healthy lifestyles, and preventive measures.
- Cultural Sensitivity: Demonstrate cultural competency and sensitivity to cater to the diverse needs and backgrounds of the community's population. Effectively communicates with diverse populations, including physicians, employees, patients, and families.
- Documentation: Maintain accurate and up-to-date electronic health records (EHR) to document patient encounters, treatment plans, progress notes, and other essential information. Alternates monthly on call and documents phone encounters in the medical record. Participates in clinical staff meetings and multidisciplinary team conferences as needed.
- Community Engagement: Participate in community outreach and health promotion initiatives to raise awareness and enhance access to healthcare services.
- Quality Improvement: Contribute to continuous quality improvement efforts by participating in performance evaluations, audits, and staying updated on the latest healthcare guidelines and best practices.
Qualifications:
- Master's or Doctoral degree in Nursing (MSN or DNP) with Family Nurse Practitioner specialization.
- Active Texas Advanced Practice Registered Nurse (APRN) license and national certification as a Family Nurse Practitioner.
- Proficiency in conducting physical exams, diagnosing conditions, and developing treatment plans.
- Strong communication skills to effectively interact with patients, families, and healthcare team members. Must have the ability to exercise sound judgment and discretion.
- Understanding of the healthcare needs and challenges faced by suburban and rural communities.
- Ability to adapt to a fast-paced and dynamic healthcare environment.
- BLS Certification Required.
