Job DescriptionJob DescriptionCase Manager/Care Manager for Joint Base Andrews, MD. 
Licensed Clinical Social Worker (Independent) provides case management, care coordination, and discharge/disposition planning for both inpatient and outpatient settings.

Case Management Duties: 

  • Assists service members, veterans, and their families in accessing appropriate healthcare options for a variety of conditions including acute, chronic, complex, and catastrophic illnesses, as well as mental health issues like combat stress and traumatic brain injury adjustments.
  • Coordinates care across multiple providers and care levels, addressing both psychosocial and medical needs.

Assessment Responsibilities:

  • Proactively identifies patients and families for case management from various sources, including discharge planning, referrals, and Medical Evaluation Board (MEB) processes.
  • Conducts comprehensive assessments encompassing the patient’s physical, emotional, psychological, social, and medical status through direct contact and information from caregivers and medical records.
Planning:
  • Develops tailored care plans with short and long-term goals, coordinating with patients, families, primary providers, and the healthcare team.

Implementation:

  • Guides patients and families through the healthcare system, enhancing resource utilization.
  • Executes care plans, coordinating all necessary referrals and advocating for patient education
Coordination:
  • Manages care delivery in various settings, including alternate healthcare environments and home settings, to improve health, safety, and quality of life.
  • Develops and maintains connections with military and community resources to optimize service access.
Monitoring and Evaluation:
  • Regularly updates treatment plans and maintains documentation as per Department of Defense (DoD) and other relevant guidelines.
  • Monitors treatment adherence and evaluates outcomes, identifying any deviations from standard practices.

Additional Duties:

  • Travels as needed for patient advocacy and to ensure appropriate living conditions.
  • Participates in interdisciplinary team meetings, sharing insights from clinical practice and social work.

QUALIFICATIONS OF THE SOCIAL WORKER CASE MANAGER:
  • Degree/Education: Master of Social Work (MSW) degree from an accredited school of social work.
  • Certifications in addition to Basic Life Support: Certified Case Manager by Commission for Case Management (CCM) or National Association of Social Workers (NASW)
-OR-
  • certification eligible with 2 years of full-time experience in case management for adults, children, families, seniors, and groups.
Experience:
  • Minimum of 2 years full-time experience in professional case management or discharge planning programs for adults, children, families, seniors, and groups.
  • Must have the knowledge to effectively apply the following core case management functions:
    • a) Assessment: Identification of patients for case management;comprehensive collection of patient information and medical status;and continued evaluation of an established plan of care;
    • b) Planning: Collaboration with the patient, family/caregiver, primary provider and other members of the health care team for developing an effective plan of care;
    • c) Facilitation: Care coordination and communication among all involved parties;and
    • d) Advocacy: Support for the patient and family/caregivers to ensure identified education and appropriate, timely care is received.
  • Must be knowledgeable in theories, principles, and methodologies underlying psychosocial practice;developmental growth;dynamics of human behavior, family, and other social systems;and the impact of illness and disability on social functioning;medical privacy and confidentiality (Health Insurance Portability and Accountability Act [HIPAA]), and accreditation standards of Accreditation Association for Ambulatory Health Care (AAAHC) and The Joint Commission (TJC);and computer applications/software to include Microsoft Office programs, MS Outlook (email), and internet familiarity is required.
  • Must be skilled in program planning, implementation, and evaluation, and in conducting individual, family, group, and community assessments;applying critical thinking skills and expertise in resolving complicated healthcare, social, interpersonal and financial patient situations;and problem-solving to articulate medical requirements to patients, families/care givers, medical and non-medical staff in a professional and courteous way.
  • Board Certification: must be in compliance with Exhibit 5 requirements for a Social Worker-Case Manager.
Licensure/Registration: A current valid license with no adverse action pending from any state or national licensing/certification agency. A Social Worker license is defined as a license that allows the Social Worker to work independently without clinical supervision.
  • US Citizenship required (for access to government computer systems).
  • BLS for Healthcare Providers
