Job Summary:
The Nurse Practitioner works in collaboration with a physician to provide comprehensive wellness, acute, episodic, and chronic care to families and individuals across the life span within a community health center.
Essential Functions and Responsibilities:
- Provide services related to health maintenance and promotion.
- Perform physical examinations and assessment of individuals of various life spans.
- Assist with diagnosis, treatment and management of acute and chronic health conditions.
- Order, perform and interpret laboratory and radiology tests.
- Prescribe medications including controlled substances to the extent delegated and licensed.
- Consult with the collaborative physician as needed for patient care that requires extensive case management and referral (s).
- Perform other therapeutic or corrective measures as indicated, including urgent care.
- Prepare and maintain accurate patient records, charts and documents to support sound medical practice and reimbursement for services provided.
- Adhere to the practice of confidentially regarding patients, families, staff and the organization.
- Performs all other duties as assigned.
Minimum Qualifications:
Education
- Master’s of Nursing degree required.
Experience
- 6 – 12 months of experience working in a clinic as a nurse practitioner preferred.
- Pulmonary experience, required.
Certifications, Licenses, and Registrations
- CPR certification (BLS, PALS, ACLS).
- Advance Practice Nurse licensed in the state of IL.
- Controlled substance license and DEA license.
- Registered Nurse licensed in the state of IL and Board Certification (i.e. ANCC).
FLSA Statement:
This position is classified as non-exempt under the Fair Labor Standards Act (FLSA). Employees in non-exempt positions are entitled to receive overtime pay for any hours worked over 40 in a workweek in accordance with state and federal regulations.
