Requirements:
• Ability to complete visit check in and intake to record patients medical history
• Basic understanding of the patient examination process
• Measure patients' vital signs, such as blood pressure, pulse ox and temperature
• Proper knowledge on stethoscope placement to check heart and lung sounds
• Basic understanding of medications
• Assist provider with patient examinations via video meeting
• Perform testing in patients' home (EKG, ABI, Spirometry, Balance Plate)
• Basic understanding of EKG lead placement
• Administer vaccines, injections or medications as directed by physicians and as permitted
by state law and document properly in patients chart
• Valid driver's license and reliable transportation required to visit patients homes and/or
facilities
Responsibilities:
• Log into route manager application and ensure patients match your patients scheduled on
EMR
• Log into EMR review schedules and appointments notes
• Send RingCentral video link via email at the beginning of shift to assigned provider
• Contact your assigned providers to establish yourself as their point of contact for the day.
• Call all patients in advance of arrival and provide ETA
• Create patient case with patient concerns and/or requests, assigning to appropriate staff.
• Inform clinical staff when unable to perform testing or if patient is a no show
• Troubleshoot equipment and inform management immediately of any issues
• Reconcile patient medication list
• Maintain inventory of medical supplies
• Enter patient information, such as vital signs and test results, into patients chart
• Assist with wound measurements and documentation
• Upload documents and forms to patients chart for provider review
