Job Summary:

The primary responsibility of the facility-based Advanced Practice Provider (APP) is to provide direct patient care services at the Provider level within post-acute care settings.

Key Responsibilities:
- Embrace our company's care philosophy to enhance each patient's clinical status and quality of life.
- Conduct Annual Wellness Visits and other clinical assessments as required.
- Provide ongoing evaluation and treatment of chronic illnesses.
- Coordinate care with collaborating physicians and other healthcare professionals.
- Maintain communication with families and attend conferences when possible.

Qualifications:
- Must hold a valid nurse practitioner or physician assistant license and be board-certified.
- Bachelor's and Master's degrees in Nursing from accredited institutions.
- Nursing experience is required, with some preference for Nurse Practitioner/Provider experience.
- Strong leadership, interpersonal, and problem-solving skills are preferred.
- Excellent written and oral communication skills are necessary.
- Punctuality, organizational skills, and teamwork abilities are vital.
