We have a great opportunity for a PRN or Part-Time Home Health Speech Language Pathologist (SLP).

1:1 Patient Care

Service Area – Muncie, Alexandria, Kokomo and surrounding areas

Monitor carefully the patient status and progress toward goals, as well as compliance with physician’s orders. Responsible for educating patients and caregivers, while preparing the patient/caregiver for discharge as appropriate.

Job Responsibilities

  • Provide skilled speech language pathology in accordance with the physician’s plan of treatment.
  • Observe, record, and report the client’s reaction to treatment and any changes in the client’s condition to the physician, DON/ Clinical Manager, according to policy.
  • Develop, implement and revise the client’s plan of care.
  • Promote coordination of care through interdisciplinary case conferences and communication with physician and field staff.
  • Perform therapy interventions utilizing standard speech language pathology techniques and skills as appropriate for the condition of the patient.

Education, Licensure(s) and Qualifications.

  • Minimum of 2 year experience as a Speech Language Pathologist required.
  • Previous Home Health experience is preferred.
  • Must be currently licensed to practice in the state of Indiana
  • Must be CPR certified
  • Current Drivers License, vehicle insurance and access to a dependable vehicle
