Community Health Centers of America is looking for a PA/FNP to join our group. We are looking for a provider to work with us at a clinic. We would like you to join us and serve our mission to provide affordable, compassionate, and quality care to promote the well-being of all the members of the community.



Job Description


This position description has been designed to indicate the general nature and level of work performed by employees within this classification. POSITION SUMMARY: Diagnoses and treats acute, episodic, or chronic illness, independently or as part of a healthcare team.


PRIMARY ACCOUNTABILITIES:

  • Obtains, analyzes and interprets patients' histories, symptoms, physical findings, or diagnostic information to develop appropriate diagnoses.
  • Diagnoses or treats acute health care problems such as illnesses, infections, or injuries.
  • Recommends diagnostic or therapeutic interventions.
  • Prescribes medications.
  • Educates patients about self-management of acute or chronic illnesses.
  • Orders, performs, or interprets the results of diagnostic tests.
  • Maintains complete and detailed records of patients' health care plans and prognoses.
  • Detects and responds to adverse drug reactions.
  • Keeps abreast of developments in nursing.
  • Schedules follow-up visits to monitor patients or evaluate health or illness care.
  • Performs routine or annual physical examinations.
  • Supervises or coordinates patient care or support staff activities.
  • Maintains current knowledge of state legal regulations for nurse practitioner practice.
  • Keeps abreast of regulatory processes and payer systems.
  • Obtains health information from patient.
  • Performs physical examinations and assesses any medical problems.
  • Practices safe and healthy control methods while working with patients.
  • Education to patients and families concerning medical treatments, medications, etc.
  • Administers or orders for diagnostic tests such as x-ray, electrocardiograms, and lab tests.
  • Interprets test results to diagnosis any related health issues.
  • Counsel patients about compliance with medication.
  • Assists in pre-operation or post-operation care.
  • Coordinates activities of patient care.
  • Documents patients’ progress with medication or treatment.

OTHER RESPONSIBILITIES:

  • Follow Community Health Centers of America’s rules, policies, and procedures while on assignment
  • Follow OSHA safety rules and procedures while on assignment
  • Respect cultural and religious practices of patients
  • Uphold HIPAA regulations
  • Punctual and dependent for assigned/confirmed shifts
Requirements:

POSITION REQUIREMENTS:

  • Education: Master's degree
  • Experience of 3-5 years of related experience; or equivalent combination of education and experience
  • Licenses/Certifications: Appropriate certifications and/or licensures
  • Current BLS/ACLS required.
