**JOB SUMMARY**

This is a per-Diem part-time opportunity.

The Nurse Practitioner (NP) is an advanced practice registered nurse with specialized education and training, providing extensive hospice services in accordance with the state Nurse Practice Act and under the guidance of a collaborating physician. The NP offers hospice care by assessing health histories, promoting health, diagnosing medical conditions, managing health issues, developing care plans, prescribing treatments, and coordinating with patients, families, and healthcare professionals. Working under a physician’s supervision, the NP conducts patient visits, performs physical assessments, and determines hospice eligibility in consultation with the interdisciplinary team. The NP utilizes community resources to plan and direct care for individuals and families in their homes and communities.


**KEY RESPONSIBILITIES**

1. Delivers medical care, manages terminal illness, and alleviates symptoms according to state regulations and under a physician’s oversight.

2. Conducts thorough patient histories and physical exams, orders and interprets lab and diagnostic tests, diagnoses conditions, and documents findings.

3. Develops and manages individualized care plans, prescribes medications and treatments, and coordinates referrals and hospice services.

4. Performs face-to-face hospice visits to assess eligibility for continued care, providing clinical data for recertification.

5. Advises the hospice medical director on patient eligibility for initial certification and recertification.

6. Reviews patient information, coordinates care with the interdisciplinary team and other healthcare providers.

7. Ensures symptom management through collaboration with team members.

8. Develops treatment protocols and intervention options based on current literature in collaboration with the physician.

9. Consults the physician for care plans that deviate from established protocols.

**Additional Duties**

1. Stays informed about Medicare/Medicaid and state/federal regulations for hospice services.

2. Ensures adherence to Medicare conditions of participation and other healthcare regulations.

3. Complies with HIPAA requirements and organizational policies.

4. Participates in quality monitoring and improvement efforts.

5. Pursues professional development through seminars, conferences, and continuing education.

6. Completes assigned tasks and responsibilities.

7. Demonstrates expertise in communicating and counseling on end-of-life issues.

**Communication**

1. Collaborates with the interdisciplinary team to create, review, and update patient care plans.

2. Communicates with the attending physician, hospice medical director, and other healthcare providers about the patient's care needs and management.

3. Coordinates with community health practitioners for comprehensive care planning.

4. Participates in interdisciplinary group meetings.

5. Maintains accurate clinical documentation, including notes and medical orders.

6. Educates patients, families, and caregivers on preventive care, medical, psychological, and spiritual issues, working with the team to address all patient needs.

7. Ensures a safe environment for patients.

8. Assists in maintaining continuity of care with family and other team members.

9. Works with families and the interdisciplinary team to establish and meet care goals.

10. Provides consultation and education to team members and community healthcare providers.

11. Builds and maintains relationships with staff and referral sources to support program development.

**QUALIFICATIONS**

1. Graduation from an accredited nursing school.

2. Current DEA license.

3. Completion of an accredited Nurse Practitioner Program meeting board standards or certification by an equivalent national organization, or a registered nurse with an approved program.

4. Current nursing license and CPR certification.

5. Master’s degree, with at least one year of Nurse Practitioner experience and one year of hospice or palliative care experience.

6. Certification from an approved advanced practice nursing body.

7. Preferred certification in a specialty area such as Hospice and Palliative Nursing (CHPN), Pediatrics, or Geriatrics.

8. Strong observational, verbal, and written communication skills; problem-solving abilities; and nursing competencies.

9. Physical ability to perform required nursing tasks, including lifting, transferring patients, and prolonged standing or walking.

10. Valid driver’s license, insured and access to reliable transportation in compliance with state or organizational requirements.

11. Must have at least 1 year of Hospice experience
