Job Title: Community Living Specialist Department: Community Living Services (CLS)

Reports to: Community Living Advocate or Community Living Quality Coordinator

Duties/Responsibilities:

  • Meal planning, preparation, and/or serving. Encouraging water/fluid intake. Monitoring and documenting food/liquid intake and output if necessary.
  • Assist individual with eating meals/snacks, bathing tasks, oral hygiene, hair care, nail care, dressing, toileting and hygiene tasks, and positioning/repositioning in bed and/or wheelchair if necessary. Monitoring skin integrity,
  • Assisting individual with basic kitchen clean-up, vacuuming/sweeping/mopping, laundry tasks, ironing, dusting, Bed linen change, biohazardous waste clean-up if needed, etc.
  • Financial management/ budgeting assistance as needed with personal care and household shopping, community events, service providers, etc.
  • Picking up prescriptions from pharmacy, arranging trips to the doctor, and assisting with communication regarding the individuals’ health and needs as needed.
  • Transportation assistance to and from stores, restaurants, community events, medical/dental visits, etc. Ability to track mileage per company policy.
  • Taking vital signs and/or assisting with basic first aid if necessary.
  • Assisting individual with self-administration of medication or reminding to take medications if needed.
  • Reporting any changes in individual’s condition, environmental conditions/issues, and questions/concerns to the appropriate parties.
  • Completed all required documentation regarding individual care and oversight including, shift notes, ISP and SP goal-tracking, health care monitoring, medical and dental appointment information, etc. Most documentation completed in Therap.
  • Monitoring Relias system for required trainings, documentation responsibilities, and other tasks as assigned.
  • Other tasks and duties assigned by the supervisor as needed.

Required Skills/Abilities:

  • Excellent verbal and written communication skills.
  • Excellent interpersonal and conflict resolution skills.
  • Excellent organizational skills, time management skills, and the ability to work independently and prioritize tasks while adhering to new and established deadlines.
  • Strong problem-solving skills
  • Proficient with Microsoft Office Suite or related software. Ability to learn new computer-related skills specific to documentation, communication, and training responsibilities in Therap, Relias, ADP, AOD, Caregiver Plus (EVV), etc

Required Certifications and Licenses: Certified Nursing Assistant, Patient Care Assistant, and other similar trainings/certifications are preferred but not mandatory. First Aid, CPR and CPI (Nonviolent Crisis Prevention Intervention) certification required.

Education and Experience:

High school diploma or equivalent.

At least two years related experience working with individual with DD and/or ID required.
