Job Summary:
The Southeastern Integrated Care Patient Care Flow Nurse assesses, directs, manages, and evaluates the delivery of nursing care and patient support services daily to achieve a specified standard of nursing care for our designated patient population. Ensures the existence of streamlined patient care delivery systems, which promotes smooth patient transitions across the patient care continuum. Through effective fiscal and operational management, maintains optimum patient care unit systems, including clinical staffing, clinical scheduling, and communication systems. Assures ongoing education, and development programs are available to expand each clinical staff member’s (nurse, FNP-C, PA-C etc.) individual capacity for growth.

Essential Responsibilities and Duties:

  1. Monitor and Manage Patient Flow: The patient care flow nurse oversees patient admissions, discharges, and transfers to optimize and ensure efficient patient flow within the facility.
  1. Implement Policies and Procedures: They implement and oversee patient placement policies and procedures to comply with healthcare regulations and standards. Will provide supervision from time to time for Medical Assistants and LPN as requested by Director of Practice Management
  1. Analyze Data and Prepare Reports: The patient care flow nurse analyzes patient flow data and forecasts provider and clinical staff availability.
  2. Facilitate Communication: Facilitate communication and coordination between behavioral health and primary care providers to ensure seamless integration of services and promote holistic patient care.
  3. Train and Mentor Staff: New clinical staff members receive training and mentoring on patient flow procedures and the use of patient management software systems.
  4. Coordinate Care: Collaborate with behavioral health providers, primary care physicians, and other healthcare professionals to develop and implement integrated care plans for patients with complex medical and behavioral health needs. Supports referral coordinator with medical and behavioral referrals as needed. Supports LPN and Medical Assistants at the direction of the Director of Practice Management
  5. Chronic Care Management: Provide ongoing support and education to patients with chronic conditions, including monitoring medication adherence, lifestyle modifications, and self-management techniques.
  6. Documentation: Maintain accurate and up-to-date electronic health records (EHR) and documentation of patient encounters, care plans, interventions, and outcomes. Provides medication reconciliation assistance as needed.
  7. Quality Improvement: Participate in quality improvement initiatives to enhance the delivery of integrated care services and improve patient outcomes within the facility. Supports quality metric adherence.
  1. Professional Development: Stay updated with current trends and evidence-based practices in integrated care, behavioral health, chronic disease management, and remote patient monitoring through continuous education and professional development activities.

Qualifications and Education:

  • Licensed Practical Nurse, (LPN) or Registered Nurse, (RN) in the state of North Carolina REQUIRED
  • Bachelor of Science in Nursing (BSN) degree PREFERRED
  • Minimum of 2 years of clinical experience in an integrated care or primary care setting REQUIRED
  • Minimum of 2 years in a leadership/management position PREFERRED
  • Experience in coordinating care between behavioral health and primary care providers
  • Strong knowledge of chronic disease management principles and evidence-based practices
  • Familiarity with remote patient monitoring technology and its application in healthcare PREFERRED
  • Excellent communication, interpersonal, and collaboration skills.
  • Ability to work effectively as part of an interdisciplinary care team.
  • Proficient in using electronic health record (EHR) systems.
  • Commitment to providing patient-centered and culturally sensitive care.
