Job Summary

Implements OCHN’s strategic vision and plan in support of the Behavioral Health Home model, including initiatives and projects that: 1) improve care management of beneficiaries; 2) improve care coordination between physical and behavioral health care services; and 3) improve care transitions between primary, specialty, and inpatient settings of care. Acts as primary point of contact and OCHN lead for Health Home Partners, MDHHS, and other stakeholders. Coordinates and leads OCHN’s interests by facilitating and/or administration of local, state, and national collaborative projects to develop/refine policy and procedures that will further community inclusion, create efficiencies, and coordinate resources to ensure the best outcomes for those we serve.

Essential Functions

For Behavioral Health Home Programs

  • Lead the development, implementation, and management of Health Home services.
  • Provides coordination of health home activities
  • Collects, analyzes, and reports on data that permits an evaluation of increased coordination of care and chronic disease management.
  • Monitors Health Home Partner performance and leads quality improvement efforts.
  • Designs and develops prevention and wellness initiatives, and referral tracking.
  • Executes enrollments and disenrollments using the MDHHS electronic enrollment system.
  • Provides training and technical assistance to internal and external stakeholders.
  • Engages in data management, reporting, and population health management activities.
  • Participates in other OCHN integrated healthcare activities as assigned.

Administration and Collaboration

  • Provide population group content expertise and guidance to the Provider Network for the development and improvement of OCHN service initiatives, projects, and programs. This includes research, community input, management report analysis, etc.
  • Participate in workgroups and committees with other OCHN Teams as assigned.
  • Advance and implement the principles and practices of Recovery, Self-Determination, Trauma-Informed Systems, Person / Family Centered Planning, and Best Practices, including the evaluation of outcomes and satisfaction surveys, as assigned.
  • Implement Advance Peer-delivered services among Health Home Partners.
  • Monitor Health Home Partner capacity and staffing and identify capacity building needs among the Health Home Partners.
  • Implement practices to address co-occurring disorders, including mental illness and substance abuse, as well as mental illness and developmental disabilities.
  • Implement training to ensure that providers and staff have the necessary skills to advance OCHN’s strategic plan.
  • Implement identified initiatives and projects related to local, state, and federal collaborative activities.
  • Analyze data / management reports and provides recommendations about the need for Provider improvement plans as well as recognition for exemplary practices.
  • Performs other duties as assigned.

Job Requirements and Qualifications

Education:

  • Bachelor's degree in relevant discipline required.
  • Master's degree in relevant discipline preferred.

Experience Requirements:

  • Minimum of 2 years relevant experience in human services administration, contract management, or related area, preferably in the public sector.
  • Experience in qualitative and quantitative research methods.

  • Demonstrated experience in quality assurance and quality monitoring.
  • Demonstrated experience in the preparation and monitoring of budgets.

Preferred competencies:

  • Working knowledge of the Michigan Mental Health Code.
  • Understanding of MDHHS Behavioral Health Home Handbook and requirements
  • Understanding of Medicaid rules, regulations, and Michigan Medicaid Provider Manual.

Additional Information

(Travel required, physical requirements, on-call schedules, etc.):

  • Must have available means of transportation to and from OCHN and for required offsite meetings or site visits.
  • Must be available for meetings and events which may occur outside of standard office hours.
  • Work performed primarily in an office environment.
  • Hybrid (onsite/remote) work schedule available.
