Job Summary: The Extreme Family Finder assists in helping locate family and resources for foster children. 

 

Responsibilities: Primary responsibilities include but are not limited to: 

  • Carry out recruitment strategies and meet outcome requirements. 
  • Follow requirements and policies of agency, contracts, state regulations and COA standards. 
  • Complete home studies in a timely manner, not exceeding deadlines.  
  • Ensure all biweekly statistics are completed by scheduled supervision every two (2) weeks.  
  • Maintain strong, open communication with direct supervisor regarding services delivered, concerns or questions. 
  • Maintain up-to-date documentation of client contact and progress in OneNote and internal database program, SAM. All documentation should occur within two (2) days of family contact.  
  • Maintain frequent contact with case managers of children placed in resource provider homes to ensure quality of services and resolve issues/concerns timely as they arise.  
  • Case record review: mine the file for names, phone numbers, and sometimes, social security numbers of kin/relatives.  
  • Assessment: ensure that an initial assessment of adoption readiness is conducted and updated as the case progresses.  
  • Adoption preparation (weekly): review the youth’s current placement, mental health needs, physical health needs, adult relationships, peer relationships, and educational needs.  
  • Network building (weekly): attend meetings with direct supervisor, case manager, case manager supervisor, CASA, GAL, DJO, residential staff, etc.  
  • Participate as an active team member with all agency departments (i.e. attend and participate in all scheduled internal meetings as assigned, and professional development trainings). 
  • Travel required to conduct home visits and attend meetings within counties served, with or without accommodation. 

 

Qualifications & Preferred Skills 

  • Bachelor’s Degree (master's degree preferred) in social work (preferred) or in related human services field. 
  • Previous experience working with at-risk populations required.   
  • Demonstrated resourcefulness in setting priorities and proposing new ways of creating efficiencies. 
  • Outstanding organizational and planning skills.  
  • Solid commitment to the principles of support and advocacy for foster and adoptive families. 
  • Collaborate and advocate for positive outcomes, using excellent interpersonal skills in multi-disciplinary, diverse, and dynamic professional teams.  
  • Strong verbal communication skills and demonstrated capacity to write clearly and persuasively.  
  • Demonstrated leadership, team management, and interpersonal skills. Must quickly establish rapport and build trust.  
  • Flexible and a self-starter; able to multi-task while also being highly detail-oriented while meeting all deadlines.  
  • Work as a necessary part of a team effort with colleagues of all demographics.  
  • Model ethical and professional behavior at all times. 
  • Experience working with marginalized communities with a nuanced understanding of systemic oppressions. 
  • Flexibility and adaptability, able to shift styles to fit the needs of a wide range of cultures, people, and organizations.  
  • Flexible in work hours including some evening and weekend appointments.
