MySkye Care is a Home and community-based services (HCBS) provider. Home and community-based services opportunities for Medicaid beneficiaries to receive services in their own home or community rather than institutions or other isolated settings. These programs serve a variety of targeted populations groups, such as people with intellectual or developmental disabilities, physical disabilities, and/or mental illnesses. Providing Individualized Home Supports services will help develop, maintain and improve the community-living skills of a person.

Job Role


IHS training is direct training from a staff person to address the identified skill development needs of a person in the areas of:


  • Communication skills
  • Community living and mobility
  • Interpersonal skills
  • Reduction/elimination of maladaptive behavior
  • Self-care Sensory/motor development involved in acquiring functional skills.
  • Assistance and supervision may occur during the delivery of ILS training services.
However, training must be the primary service provided.


Accountabilities


  • Maintain an up-to-date and comprehensive knowledge on all related policies and procedures, rules and regulations for related to Maltreatment of Minors Act, Vulnerable
  • Adult Act, Data Privacy, Documentation, Individual Rights, Bloodborne Pathogens and, Fraud Prevention.
  • Manage and maintain all required documentation. Prepare weekly support plan.
  • Responding to and reporting incidents as required for HCBS. Following safety practices established by MySkye Care.
  • Exercise discretion, judgment, and initiative regarding problems and inquiries.
  • Maintain confidentiality of client information. Follow established policies and procedures and do not use or disclose protected health information other than as permitted or required by the Agreement or as required by law.
Qualifications


  • High School Diploma
  • 1+ years prior experience preferred.
  • Excellent verbal and written communication skills.
  • High attention to detail, accuracy and thoroughness.
  • Ability to react quickly, under high pressure with minimal supervision.

  • Clean driving record
  • 18 years of age or older
Job Type: Part-time
Location: 2 female clients, 1 in St.Paul and 1 in Maplewood

Pay: $19.00 to $21.00 per hour depending on experience.
