Job Title: Patient Care Coordinator

Overview:

We are currently seeking a detail-oriented and empathetic individual to join our healthcare team as a Patient Care Coordinator. This role involves providing essential support in the patient care process, from welcoming patients to the clinic, gathering their medical history, and ensuring seamless communication between patients and healthcare providers. The successful candidate will play a vital role in prepping examination rooms, maintaining accurate medical records, and facilitating the overall efficiency of clinic operations.

Responsibilities:

  • Patient Intake and Medical History -
    1. Welcome and assist patients upon arrival, ensuring a positive and comfortable experience.
    2. Collect patients' medical history and chief complaints.
    3. Input patient data accurately into the Electronic Medical Records (EMR) system.
  • Room Preparation:
    1. Prep examination rooms before the doctor's arrival, ensuring cleanliness and availability of necessary supplies.
    2. Arrange and organize equipment and tools needed for patient examinations.
  • Chart Preparation:
    1. Review patient schedules and prepare charts in advance of clinic days.
    2. Ensure that all necessary patient information, medical history, and relevant documents are readily available for healthcare providers.
  • Scribing Notes:
    1. Assist healthcare providers during patient visits by accurately documenting medical notes in real-time.
    2. Transcribe dictated information, maintaining precision and attention to detail.
  • Inventory Management:
    1. Monitor and manage inventory levels for medical supplies and equipment.
    2. Place orders for supplies as needed, ensuring adequate stock levels are maintained.
  • Patient Communication:
    1. Initiate phone calls to patients for appointment reminders, follow-up, and relay important information.
    2. Address patient inquiries and provide assistance in navigating healthcare processes.
  • Collaboration with Healthcare Team:
    1. Collaborate with doctors, nurses, and administrative staff to facilitate efficient patient care.
    2. Brief healthcare providers on patients' medical history and any relevant information before their arrival.

Qualifications:

  • High school diploma or equivalent; additional education or training in healthcare administration is a plus.
  • Proven experience in a healthcare setting, with a focus on patient care coordination.
  • Familiarity with Electronic Medical Records (EMR) systems.
  • Strong organizational skills and attention to detail.
  • Excellent communication and interpersonal skills.
  • Ability to maintain confidentiality and adhere to healthcare privacy regulations.
