Your role will involve assessing psychosocial needs, coordinating community resources, offering emotional support, and advocating for patients' well-being.

Responsibilities:

  1. Psychosocial Assessment:
    • Conduct comprehensive psychosocial assessments to identify patients' and families' emotional, social, and financial needs.
    • Evaluate factors impacting patients' health and ability to adhere to treatment plans.
  2. Care Planning and Coordination:
    • Collaborate with the interdisciplinary team to develop individualized care plans that address patients' psychosocial needs.
    • Coordinate services and resources to support patients in managing their healthcare and achieving optimal outcomes.
    • Provide counseling and support to patients and families coping with chronic illness, disability, or end-of-life issues.
  3. Advocacy and Education:
    • Advocate for patients' rights and access to community resources, including financial assistance, housing, and transportation.
    • Educate patients and families about healthcare options, advance directives, and available support services.
  4. Crisis Intervention:
    • Respond to crises or emergencies affecting patients and families, offering immediate support and intervention as needed.
  5. Documentation and Communication:
    • Maintain accurate and thorough documentation of all patient interactions, assessments, and interventions.
    • Communicate effectively with healthcare team members, including physicians, nurses, therapists, and other providers, to ensure coordinated care.
  6. Quality Improvement and Compliance:
    • Adhere to regulatory requirements, policies, and procedures related to social work practice in home health care.
    • Participate in quality improvement initiatives to enhance patient care and outcomes.

Qualifications:

  • Master's degree in social work (MSW) from an accredited program.
  • Current state licensure or certification as a Licensed Clinical Social Worker (LCSW) 
  • Strong assessment, counseling, and crisis intervention skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Compassionate and empathetic demeanor with excellent communication skills.
  • BLS Certification, required.
