Job DescriptionJob Description

We are looking for an Advanced Practice Clinician, Nurse Practitioner or Physician Assistant, to provide exceptional care to patients in our walk-in clinic setting. This position is responsible for providing direct patient care including obtaining histories, examining patients, ordering appropriate labs and x-rays, performing minor procedures, discussing treatments with patients and following up, and interacting regularly with other healthcare providers.

Typical Schedule

0.5 FTE

Two, 10-hour shifts per week with a rotating Saturday shift

Qualifications

  • Graduate of an accredited school or nursing or physician assistant program, required.
  • Licensed or ability to obtain Wisconsin APRN license or PA-Certification, required.
  • Ability to obtain and/or maintain Wisconsin DEA certification and National Provider Identifier (NPI), required.
  • Ability to meet criteria/qualifications for Credentialing and Clinic Privileges, required.
  • ACLS/BLS, required.

Responsibilities include:

  • Providing direct patient care in a clinic, outpatient setting.
  • Reviewing patient's medical history while collecting, prioritizing, and synthesizing comprehensive data pertinent to the patient's health or situation.
  • Evaluating, interpreting, and coordinating information to make decisions and recommendations regarding patient care.
  • Providing education, consultation or intervention to patients (treatment plan) and their families.
  • Working collaboratively with patients to improve their health and well-being.
  • Practicing in a thoughtful manner including consideration for cultural diversity, individual autonomy and legal responsibilities.
  • Charting in electronic format in accordance with licensure and scope.
  • Actively participating in multidisciplinary team approach to case management.
  • May participate in peer review and quality assurance.
